If you have medical conditions having a significant impact on your life which are expected to last longer than six months we can develop a Care Plan which spells out your conditions, goals, needs and pathway. Care Plans may make you eligible for a rebate for up to five allied health visits per year, leading to considerable cost savings for necessary treatments. Please note that there are clear rules around Care Plan eligibility which we must follow.
